Major challenges for 200 companies affected by Putra Heights blaze: FMM
An investigation by the Occupational Safety and Health Department found that a gas pipeline explosion in Putra Heights was caused by ground instability. The soft, moist soil beneath the pipeline failed to provide adequate support, leading to cyclic loading, stress, and fatigue on the pipe.
This resulted in a ductile failure, causing a gas leak that ignited and exploded. The pipeline met technical standards, and no sabotage or negligence was involved, as confirmed by Selangor police chief Comm Datuk Hussein Omar Khan. Investigations ruled out surface activities or contractor work as causes, attributing the failure solely to the weak ground support.
